Voter Registration Form
The Florida Voter Registration Application Form is used to register to vote in the State of Florida.
You can use an online form found on the Florida Department of State - Division of Elections website to
begin the process of registering to vote in the State of Florida, or for making any change to your
current voter's record.
Notice: In order to register to vote in Florida, a citizen must swear or affirm an oath and sign
the voter registration application form. The online form must be downloaded, signed and delivered
to the Elections Office. If a voter registration applicant fails to provide any of the required
information on the voter registration application form, the supervisor will notify the applicant of the
failure by mail within five (5) business days after the supervisor has the information available in the
voter registration system. The applicant shall have an opportunity to complete the application form
to vote in the next election up until the book closing for that next election. If the applicant fails
to complete his or her voter registration application prior to the date of book closing, 29 days prior to an
election, then such applicant shall not be eligible to vote in that election.
